Miss Roslyn Investigation report: Ineffective hull inspection and maintenance leads to capsizing

NTSB published its report on the capsizing of the towboat 'Miss Roslyn', on October 9, 2018. The towboat began to flood as well as list to starboard, eventually sinking. No injuries were reported, but an oil spill was reported. When the vessel got underway, it headed downbound towards the Upper Reserve Anchorage, hugging the right descending bank. A little later, the captain noticed a persistent list that was “more than the normal starboard list,”. He claimed that the list was because the 'Miss Roslyn' was “top-heavy,” and he made several round turns to starboard to attempt to mitigate the list. Case study: Fatal Accident on Board

The following case study, based on an investigation report issued by Transport Malta, refers to a fatal injury occurred as the crew member got trapped between the mooring rope on the winch drum and the deck on the poop deck. The safety investigation revealed that in order to perform the task of stowing the mooring ropes on the winch drum, the crew members had to engage physically with the ropes while the winch was rotating.
