Transport Malta's MSIU issued an investigation report on the inadvertent release of the Free Fall Lifeboat (FFLB) and subsequent injuries to the chief officer onboard the tanker Giovanni DP, in March 2019. The investigation revealed that the crew were not fully aware of the correct procedure for testing the release of the hook.

NTSB issued an investigation report on the flooding and sinking of the commercial fishing vessel Pacific 1 while in the Bering Sea, Alaska, in February 2019. The report identified the master's decision to remain at sea with continuous flooding as key cause of the sinking. 

The Transportation Safety Board of Canada (TSB) issued an investigation report on a lifeboat release hook failure during a drill on the passenger ferry 'Northern Ranger' in October 2017, which led to minor injuries. The failure was linked to poor compliance with SOLAS and operator's manual. 

New Zealand's TAIC issued an investigation report on the contact of the jet boat 'Discovery 2' with Skippers Canyon wall while on the Shotover River in February 2019. The incident showed that jet boating is a high-risk activity that leaves very little margin for error when navigating at high speeds in narrow channels and rivers.

NTSB issued an investigation report on the flooding and sinking of towing vessel 'Tom Bussler' on Tennessee River in January 2019. The incident stressed issues associated with lack of an effective hull maintenance and repair. The incident On January 7, 2019, about 2030 local time, the towing vessel Tom Bussler was transiting in light boat condition (no tow) upbound on the Tennessee River at mile 15 near Calvert City, Kentucky, when it began flooding and quickly sank in the channel. Both crew members aboard abandoned the vessel into the river and were rescued by a Good Samaritan vessel. No pollution or injuries were reported. Damage to the vessel was estimated at $297,368, and it was scrapped.   Probable cause The National Transportation Safety Board determines that the probable cause of the flooding and sinking of the tugboat Tom Bussler was the company’s lack of an effective hull maintenance and repair program, which resulted in flooding into the bow voids and engine room through fractures in the hull.   Analysis The vessel had last been drydocked in January 2018, about a year before the accident. The UK MAIB launched a new investigation report concerning a collision between the ro-ro passenger ferry Red Falcon and the moored yacht Greylag which was sunk on its mooring as a result.

Ireland's MCIB issued an investigation report on the fire and loss of MFV Suzanne II, east of Arklow, in May 2019. While the exact source of the fire is unknown, the report highlights that the quick response of the crew and their knowledge on when to abandon ship was key to their successful rescue. 

The cruise ship Norwegian Epic contacted two mooring dolphins while docking at San Juan Cruise Port Pier 3. The NTSB issued an investigation report on the incident identifying lack of coordination between the master and pilot as key cause of the accident. 

While underway in March 2019, the heavy lift vessel Hawk  collided with a barge and a destroyer on Mississippi river. The NTSB issued an investigation report on the collision, identifying exceeding speed as key cause of the accident. 

A Deck Cadet lost his life in an enclosed space onboard the RMI-registered bulk carrier LA DONNA I, in August 2018. The Republic of the Marshall Islands issued its investigation report on the accident, identifying lack of familiarity with procedures as contributing factor to the fatality.
