The second edition of Safety Digest outlines a number of incidents and marine casualties. A marine incident included, which will be explained below, concerns the collision of a gas carrier and a container vessel.

A trailing suction hopper dredger collided with a products tanker that was anchored at Malacca Anchorage, Malaysia, on October 2.

Ireland's MCIB issued an investigation report on the collision between the tanker 'Varkan Ege' and the sailing vessel 'Medi Mode' in Irish waters in August 2019. The investigation attributed the incident to a mix of misunderstanding, inefficient lookout and poor knowledge of COLREGs.  

A Hong Kong registered oil tanker collided with two fishing vessels in the waters of Japan Sea. Due to the collision, one of the fishing vessels capsized with a total of 13 lives lost aboard. 

On Wednesday 24th September, a Russian navy vessel collided with a Swiss-owned container ship in Danish waters off the Oresund Bridge linking Denmark and Sweden, as Danish defence authorities told Reuters.

RMI issued an investigation report on the collision of the container ship CAPE MORETON with the fishing vessel MIN XIA YU 04555 in January 2019, which caused two fatalities. The investigation highlighted failure of both vessels to take adequate action to avoid collision. 

A crane worker was injured after the giant container ship 'MSC MIA' collided with a gantry crane while manoeuvring at the Spanish Port of Valencia, in the morning hours of Sunday.

JTSB issued an investigation report on the collision of the car carrier GLOVIS COMPANION with the fishing vessel HIGASHIDA MARU, in September 2019. The incident stressed the need for vessels crossing a traffic route to enter the route after fully ascertaining the passage of other vessels navigating in the area. 

Transport Malta's Marine Safety Investigation Unit (MSIU) issued an investigation report on the collision of the Maltese registered oil tanker KANALA and the Chinese registered fishing vessel LIAODANYU 23626 on the Bohai Sea, China, in September 2019. 

The National Transportation Safety Board issued Thursday four safety recommendations in its final investigation report of the fatal collision between the US Navy destroyer Fitzgerald and the container ship ACX Crystal. These include the need of training on collision regulations, as well as review and revision of bridge resource management training.
