Belgium's Federal Bureau for the Investigation of Maritime Accidents (FEBIMA) has issued an investigation report on a serious injury of a crew member onboard the bulk carrier MINERAL TEMSE, while at the South Atlantic Ocean, on 5 May 2020.

Misinterpretation of navigational information led to the grounding of the Royal Canadian Marine Search and Rescue (RCMSAR) vessel 'Spirit of Sooke' near Sooke, British Columbia, said the Transportation Safety Board of Canada (TSB) in its investigation report on the incident. 

Transport Malta's MSIU issued an investigation report on two man overboards from the bulk carrier MV Stara Planina, in January 2020. The two crew members were lost overboard when unexpected waves washed over the poop deck, as they were not secured to the vessel.

Transport Malta issued an investigation report on a serious injury to a crew member in the grey water tank of MV LA SOLOGNAIS while underway in December 2019. The report revealed that a wiper, who entered the vessel’s grey water tank, suffered from the effects of possible hydrogen sulphide inhalation.

UK MAIB issued an investigation report on the fatality of a crew member in an enclosed space onboard the fishing vessel Sunbeam off Scotland, in August 2018. The report highlighted that entering enclosed spaces without safety precautions had been normalised by the crew through many years’ operations.

NTSB issued an investigation report on the contact of tow William C with a Rock Island railroad bridge protection cell, on Des Plaines River, in January 2020. The investigation established that high speed prevented the pilot to correct the tow’s position after completing the transit through the previous bridge. 

In its latest report, UK Maib describes the fatal crush accident onboard the general cargo vessel "Karina C"at Seville, Spain back in May 2019.

Transport Malta issued an investigation report on the fatal fall of a crew member onboard the bulk carrier MV Alma while in the Atlantic Ocean in heavy weather. While there were no witnesses, the investigation did not exclude the possibility that the crew member either lost his stepping or balance due the rolling and pitching motions of the vessel.
