RMI issued an investigation report on the fatal fall from height of a crew member onboard the bulk carrier SUN MASTER, in June 2020. The investigation highlighted a lack of situational awareness and safety barriers as main contributing factors to the incident. 

The LNG carrier ‘Genesis River’ collided with a 297-foot-long tank barge being pushed ahead by the 69-foot-long towing vessel ‘Voyager’, spilling petrochemical cargo into the Houston Ship Channel and capsizing a barge.

Following EVERGREEN statement on March 29, that Ever Given was successfully refloated within the Suez Canal at approximately 15:00 local time, the vessel will now head to the Great Bitter Lake in Egypt for a full inspection and experts will seek to investigate what caused the grounding.

On March 29th, Evergreen Line confirmed that EVER GIVEN has been successfully refloated within the Suez Canal at approximately 15:00 local time.

The Ever Given has been “successfully refloated” on the Suez Canal, according to Inchcape Shipping Services, a week after it ran aground in high winds blocking one of the world’s busiest shipping lanes.

For the sixth consecutive day, the megaship Ever Given remains stuck in the Suez Canal, after grounding on Tuesday 23rd of March, leaving more than 300 ships, including dozens of container ships and bulk carriers, unable to use the key trading route. Salvage efforts are ongoing while progress depends on wind conditions and tides. On 28 March, the Egyptian President, Abdel Fattah el-Sisi, ordered preparations for lightening the ship's cargo.

Following the recent grounding incident of the large container ship "Ever Given" in Suez Canal, the vessel's company confirmed that crew, ship and cargo are all safe, while no marine pollution has materialized.

NTSB issued an investigation report on the flooding of the towing vessel 'Alton St. Amant' while moored in the Harvey Canal in New Orleans, leading to oil discharge into the water. The investigation identified lack of pre-inspection and monitoring procedures for water transfer as a key cause of the incident. 

Belgium's FEBIMA issued an investigation report on the collision between the cargo ship MV ACEROMAR and the fishing vessel Z.575- HEIN SENIOR in the traffic separation scheme Terschelling - German Bight near Schiermonnikoog which led to material and structural damage to both vessels. 

JTSB issued an investigation report on the collision of the container ship APL GUAM with the the container ship MARCLIFF and subsequent collision of the latter with the anchored container ship HANSA STEINBURG, off Keihin Port, in March 2019.
